Shift scheduling — outcome review template
Compare intended and actual results after you collect availability and resolve time clashes. Keep a short improvement note for the next time you do the work.
When this template is useful
Use this after the task or review period, when you can compare the intended outcome with the actual result.
- Use a specific entry such as “Collect availability” so someone picking up the record can understand what was checked.
- Keep the result of “Resolve time clashes” with its supporting reference. Separate work that is complete from anything still waiting for a reply.
- Record agreed owners and dates. When plans change, a short explanation beside the original entry preserves the useful history.
How to complete this record
Compare proposed coverage with the people available for each shift.
- Choose the period or task being reviewed and the intended result. Use the activity record for supporting detail rather than copying every row.
- Compare actual outcomes with the intended result. Keep an unresolved item visible and describe the evidence behind the assessment.
- Write one practical change for the next attempt and the person who should receive it. A review is useful when it changes the next plan.
A mistake to avoid
A draft rota must not be confused with approved leave or an accepted shift change.
What a useful finished record shows
The current rota identifies coverage gaps and who confirms changes.
Keep supporting documents in the location approved for your task. Include an understandable reference in the record; do not paste confidential documents or access credentials into a broadly shared copy.
Choose the right record for this task
These are five different stages of the same workflow. Use the stage you need; you do not have to download all five.

| Resources checklist | Check that the inputs and access are ready. |

Choose the right download
| Format | Best for | Before sharing |
|---|---|---|
| Printing on US Letter and writing by hand. This is a printable document, not an interactive PDF form. | Print the blank form and any needed continuation; leave out the example and guide. | |
| Excel XLSX | Typing into a spreadsheet. Blank, continuation and example sheets are separate. | Check the print preview and any supplied formula ranges before sharing. |
| Editable HTML | Typing into the downloaded file in a modern browser; no account required. | Use Save edited copy before closing. Print / Save PDF uses your browser's print dialog. |
HTML entries are not uploaded to this site. Save the edited file somewhere you can find it, because closing or refreshing before saving can lose changes. PDF and HTML editions do not calculate totals.
Spreadsheet calculations
This spreadsheet is an editable record without built-in calculation formulas.
Where supplied, formula cells are marked separately from input cells. If you add rows or change the structure, check that the formulas still cover the records you intend to include.
Questions before you start
What should I double-check for this task?
A draft rota must not be confused with approved leave or an accepted shift change.
What if I need more space for shift scheduling — outcome review?
Use the continuation included with the download. Add the main document reference and sheet number so the extra entries remain connected to the original record. Keep the same date and unit conventions.
Should I include the filled example when sharing?
No. Share your completed blank form and the continuation pages you actually used. Keep the fictional example and guidance for your own reference.
Is this an official or jurisdiction-specific form?
This is a general-purpose organizer. If an employer, authority or receiving organization specifies a form, use its current requirements. This download does not establish legal, tax or professional compliance.
